USAPL Athlete Location Form
The following forms are for providing detailed contact information of all USAPL World Team Members to help admistration with performing Out of Meet tests. Forms are to be filled out, then faxed, emailed or mailed to the Team Managers, Drug Testing Chair and National Office no later than the date indicated on the form. Newly appointed team members must submit the Athlete's Location Form upon acceptance of a team slot.
