USAPL Athlete Location Form (ALF)
The following forms are for providing detailed contact information of all USAPL Team Members or Arnold Sports Festival participants to help admistration with performing Out of Meet tests. The ALF Form is to be filled out online and will automatically send the information to the ALF Chair and send you a confirmation copy for your files.
ALF Forms are to be submitted prior to the start of each quarter, or when you accept a team slot, or if there are any changes in your current whereabouts for the quarter.
